Application,
Documentation and Signatures
Download an application. The
following documentation and signatures must be attached (or included as
specified in your application) to your application at the time of its
submission or the application will not be considered for an award.
Signatures as required
within the application.
q OFFICIAL TRANSCRIPT copy from the latest academic year (only official
transcript accepted, otherwise application will be denied).
q If FAFSA results are
available, include those with the application.
q A one page essay
addressing the health care education you have chosen to pursue and/or are
continuing to pursue – how you arrived at your decision and how this
scholarship will help you meet your goals. Also, if you
are a BCH employee, discuss how your
additional education will enhance your service to BCH, or if you are a BCH
College volunteer, explain why your
service to BCH makes you a deserving scholarship recipient.
q Two letters of
recommendation from non-family members attesting to academic achievement and
goals of the applicant. Letters
must be written and dated after
January 2010 and contain the address, phone number, and email address of the
person writing the recommendation.
Interviews
All communication regarding application
acceptance/non-acceptance will be via email. Interviews for qualifying candidates will be scheduled via
an online scheduling program after the Scholarship Committee reviews all
applications. If you have any

Notification
of Award
Within two weeks of the interview, applicants will be
notified via U.S. mail whether or not an award has been granted.
Scholarship
Reception
All
scholarship recipients are invited to, and expected to attend, the Scholarship
Reception that will be held at 4:45 pm on Friday, May 7th, 2010, in
the Leo Hill Conference Room at Boulder Community Foothills Hospital at 4747
Arapahoe Avenue, Boulder. This is
a time of celebration and sharing of future plans. It is also a time to acknowledge those in the Auxiliary who
have worked to provide the funding for the scholarships.
